REGISTERING THE DEATH
Documents required
1 Medical Certificate of Death 2 Birth Certificate 3 Marriage Certificate 4 Medical Card if available
Forms which the Registrar will give you.
1 Form 14; to be given to the funeral director before the funeral can take place. 2 Green Form; to be completedand sent to the D.S.S. to cancel any pension, income support or any other benefits received from the government. 3 Extract of Death Certificate ; the registrar now issues, without a fee, an abbreviated Extract of the Death Certificate which is believed to be sufficient for the closing of bank accounts. HOWEVER, if a full Extract of the Death Certificate is required for a life insurance policy or similiar document, then a fee is payable, per copy, for supplying the necessary Extracts.
The duty to provide the information to the Registrar lies with a relative of the deceased. Registration MUST take place prior to the funeral, and neither burial nor cremation is allowed to proceed until the death is registered. Your Funeral Director will explain fully the procedure involved in registering the death.
If you are alone or simply unsure what to do, your Funeral Director can arrange for someone to accompany you to the Registrar's Office.
Professional Service:
This includes an appropriate proportion of our overheads, the preparation and care of the deceased, use of our Service and Rest Rooms, making all necessary arrangements for cremation or burial, obtaining statutory forms from doctors and hospitals, paying fees on your behalf, preparing and attending all necessary documentation, arranging obituary notices with national and local newspapers, receiving, handling and caring for floral tributes, making and receiving all telephone calls, arranging for the attendance of the minister or priest at the funeral service, providing a 24 hour service for your assistance and support up to the occasion of the funeral and providing any further advisory service required.
Other Services:
These charges will vary according to your requirements and will cover such items as: limousines, removal to church.
Disbursements: These are fees for the cremation or cemetery, minister, church fees, church officer, organist, doctors fees, newspaper notices, flowers, hymn sheets, funeral teas.
The disbursements are payable by us on your behalf and these charges are not controlled by the Funeral Director.
FLORAL TRIBUTES
The gentle beauty of flowers expresses your personal remembrance and brings comfort to the bereaved. Your Funeral Director can advise on suitability and availability on request.
DONATIONS TO CHARITY
If donations to charity are requested in lieu of flowers we will accept donations on your behalf and forward them to you so that you can send them on to the charity of your choice.
